Your personal data will be collected and processed in the following situations:
If you fill in your personal data and submit the Consent Form on this homepage;
If you apply as a user / client to the Foundation's health services and sign the Consent Statement.
For what purposes and on what grounds your personal data may be used
Your personal data will be collected and used for the purposes of:
(i) customer care within the scope of FNSBS health services;
ii) submission of health-related and FNSBS-related information on terms selected by you on the Consent Form;
iii) Recruitment and selection processes for FNSBS volunteer or employee.

The legal grounds applicable to the collection and use of your personal data for customer service and marketing purposes are your consent and / or the need for treatment to ensure that you provide the healthcare services you seek from our institution. in the most advantageous conditions for you and your family.

What personal data may be collected
Name; address; telephone contact; email.
Date and place of birth; marital status; household; personal and family history; profession.
FNSBS user number; insurer or subsystem customer number; civil identification number; Tax Identification Number; number of health user.
Health and special data (subject to collection and processing by health / social professionals subject to professional secrecy only)
(i) health data - necessary for the practice of preventive medicine, medical diagnosis and the provision of health care and treatment;
ii) Social data - If you are a beneficiary of FNSBS health services not fully paying for the services you benefit from for socio-economic reasons - Household income and expenditure data and access to previous year's IRS statement.
How we keep your personal data secure
We use a variety of security measures, including: separation of health data from other personal data and authentication tools, to help protect and maintain the security, integrity and availability of your personal data.
While the transmission of data security measures over the internet or website cannot guarantee complete security against intrusion, we and our partners and service providers make best efforts to implement and maintain physical, electronic and procedural security measures to protect your personal data in accordance with applicable data protection requirements. Among others we implemented the following:
i) Restricted computer access to your personal data based on access criteria by user profile;
ii) Storage of sensitive data in non-electronic written form, on file with restricted access to health and social care professionals under strict professional secrecy;
(iii) processing health data for statistical and research purposes anonymously;
iv) Protection of information technology systems through firewalls to prevent unauthorized access;
v) Permanent monitoring of access to information technology systems with a view to preventing, detecting and preventing misuse of your personal data.
