Your personal data will be collected and processed in the following situations:

If you fill in your personal data and submit the Consent Form on this homepage;
If you apply as a user / client to the Foundation's health services and sign the Consent Statement.

Your personal data will be collected and used for the purposes of:

(i) customer care within the scope of FNSBS health services;
ii) submission of health-related and FNSBS-related information on terms selected by you on the Consent Form;
iii) Recruitment and selection processes for FNSBS volunteer or employee.

What personal data may be collected

Personal information

Date and place of birth; marital status; household; personal and family history; profession.

Identification data

FNSBS user number; insurer or subsystem customer number; civil identification number; Tax Identification Number; number of health user.

Health and special data (subject to collection and processing by health / social professionals subject to professional secrecy only)

(i) health data - necessary for the practice of preventive medicine, medical diagnosis and the provision of health care and treatment;
ii) Social data - If you are a beneficiary of FNSBS health services not fully paying for the services you benefit from for socio-economic reasons - Household income and expenditure data and access to previous year's IRS statement.

How long do we keep your personal data?
